Who can use Homeward?

Homeward accepts the following Medicare Advantage plans: BCBS Michigan and Aetna. Homeward does not accept Original Medicare, Original Medicaid, or any other Medicare Advantage plans not listed above.

Who is a care navigator and why do I need one?

Getting healthcare can be complicated, and we know how important it is to feel your best. We’re here to help you navigate your care, make health goals and establish plans to reach them. We can help with your follow-up visits and schedule appointments. We can help you get answers about your insurance coverage.

I already have a doctor. Can I use Homeward?

Yes. Homeward works with your existing primary care doctors and specialists to help you meet your health goals. But care goes beyond a doctor’s visit, and we can also help with things like follow-up care, medication questions, insurance benefits, connecting with local services like meal delivery, organizing rides and household chores, and more.

How do I become a Homeward member?

Call us at 844-670-2273, Monday through Friday, 7 a.m. to 7 p.m. EST (TTY users, call 711), to request an appointment or learn more about our services. Or fill out the contact form and we’ll give you a call.

Does Homeward replace my existing doctors?

No, Homeward works with your existing doctors and specialists to coordinate your care. If you don’t have a primary care doctor or are looking for a new doctor, Homeward can get you connected with one.

Do I need to have health insurance to use Homeward?

Is Homeward a primary care provider?

We will not replace your existing primary care relationship. We empower you by meeting patients where they are - in their home and in the community.

Why does Homeward want to work with primary care providers?

We want to improve the health and wellness of rural Americans. We do that by working with outstanding local providers to extend their reach and get more people the care they need.

Who is a Homeward care navigator?

Our care navigator team will often come to the health plan member’s home and go through some basic medical assessments like blood pressure, weight and height. They’ll make sure the member is ready for the visit with the Homeward provider ensuring they are able to connect virtually. While they are alongside the plan member during the virtual provider visit, their primary role is to help coordinate non-clinical support—like food, transportation, caregiving, and DME—leveraging their local knowledge and community connections. They become the member’s advocate and also help them better understand their plan benefits and how to access those that are the most beneficial to the member.

How does Homeward partner with primary care providers?

We act as a clinical extension of your main provider care team. Homeward provides services to help you offer more to your patients and always connect them back to you. We can support with things like post-discharge care management and in-home safety checks, managing chronic conditions, checking blood pressure and handling A1C collection and management, as well as helping to coordinate and schedule preventive screenings like colorectal and breast cancer.

How does Homeward help address reimbursement gaps?

We can help address reimbursement gaps by working alongside you to deliver quality total care. Partnering with Homeward can help you reach financial incentives offered by payer partners or other arrangements. We can support identifying and addressing care opportunities to meet those incentives. This allows you to spend your time doing what you do best - delivering your best care to your patients.

Does Homeward provide clinical services?

Homeward will provide clinical services as necessary to support your clinical objectives for your patients in partnership with you. See reimbursement gaps question above for more detail.

For health plans

How does Homeward partner with health plans?

Homeward works with health plans to expand access, improve care, and manage costs by taking on full medical loss ratio (MLR) risk without delegation of traditional health plan services.

How does Homeward's solution impact risk coding?

Homeward's solution is all-inclusive to accurately identify and document risk coding. We will work with health plans to gather other information that would further inform an accurate risk assessment.

How does Homeward's solution impact quality and clinical outcomes?

Homeward closes gaps in care and drives improvement in key health plan and CMS metrics such as HEDIS and CAHPS measures.

How does Homeward identify high-value referral partners?

Whenever possible, Homeward leverages established high-performance networks as defined by our health plan partners. For health plans who do not have established high-performance networks, Homeward leverages our established network powered by our analytics platform.
