Welcome Visit  101

So, how does this work?
Homeward teams up with your health plan to help you get care close to home. We give extra support beyond your regular doctor’s office—like visits at home, video calls, or care at one of our clinics (if there’s one nearby).

It all starts with a Welcome Visit, where we get to know you and your health needs.

What happens at the Welcome Visit?

Once your visit is confirmed, we’ll send you an email and/or text confirmation with a link to complete pre-visit forms.
You’ll receive email and/or text reminders a week before your visit.

A Homeward care navigator will call you a few days before the visit to make sure you’re ready.
During your welcome call, your care navigator can explain your health plan benefits, such as:
  • Free gym access

  • Over-the-counter medications

If needed, they can also assist with medical equipment, including:
  • Oxygen

  • CPAP machine

A local care navigator will visit you at home.

They’ll bring a Welcome Visit packet with helpful information for you.
They’ll do a few basic health checks, like your blood pressure and height and weight.

They’ll also confirm which pharmacy you use, where you get lab tests, who your primary care doctor is, and any other doctors you see.

During the visit, your Homeward provider will go over:
  • Any check-ups or screenings you may need

  • The medicines you take

  • Any health conditions you're being treated for

  • Any allergies you have

  • Any health questions you want to ask

They’ll also talk with you about:
  • Any recent surgeries or medical procedures

  • Visits to the ER or hospital stays

  • Any upcoming or recent doctor or specialist visits

  • Your family’s medical history

We take time to listen, learn about you, and understand the support you may need.

We come to you in the comfort of your home

Our local care navigators, who live right there in your community, can come to your home and meet with you in person. And they’ll help launch the virtual provider part of your visit.

Did you know that we have actual clinics, a brick and mortar office, where you can come see us? You can schedule your welcome visit or any ongoing visits that may be needed as part of your care plan.

What’s great about this visit?

You’ll get about an hour of one-on-one time with your care team, between your care navigator and provider.
It’s a great way to get to know them, so if you need help in the future, you already have a trusted team ready to support you.

Understanding your needs, together

Your Homeward provider will also do an overall health review to see how we can best support you. This visit helps us understand your needs and gives you a chance to ask questions too.

Answers, adjustments, and second opinions

Your provider may find that you need a different medication or a change to what you're taking. You can also ask questions—like how to take your meds or use an inhaler the right way. If you’ve had a health issue with no clear answer, this is a good chance to get a second opinion.

You don’t have to manage your health alone

If you need to see a specialist, your care navigator will help you find one. Dealing with a chronic condition? We’re here to help you stay on track and feel your best.

Bridging you to care and next steps

Homeward's care team will build a bridge between us, you, and your PCP.
By the end of the visit, your care navigator will:
  • Identify any needed clinical and community support services

  • Begin working to get those services in place for you

You'll receive:
  • A summary of your visit

  • A list of any agreed-upon next steps or "to-do's"

Your care doesn’t stop after the visit

After your visit, your care navigator or provider may check in with you in the weeks ahead.
Think of us as an extra set of eyes—here to support you and work alongside your main doctor, especially after an ER visit, hospital stay, or any other health need. We’ll keep your doctor updated every step of the way.
