Recovering after a hospital stay can be overwhelming. The first 30 days are especially important — and that’s when you may need the most help. At Homeward, we’re here to support you every step of the way. We’ll help you stay on track with your recovery, so you can focus on feeling better.

A benefit of your health plan

Homeward works with health plans to offer extra support to members after a hospital stay — at no cost to you.

Your participation is completely optional, but we’re here if you need us.

What to expect when you get Homeward’s support after you’re discharged from the hospital.

Once you’re enrolled, you’ll be connected with one of our dedicated Homeward nurse care managers. Your nurse will:

  • Follow up with you by phone after your discharge.

  • Explain your care plan in simple terms.

  • Answer your questions about medications, appointments, or symptoms.

  • Coordinate with your hospital and doctor, so everyone is on the same page.

  • Connect you with helpful services such as: Transportation to follow-up visits, in-home care or nursing support, meal delivery and other resources.

A care team in your corner.

At Homeward, your care team is more than just a voice on the phone. You’ll have access to:

  • Registered Nurses (RNs)

  • Nurse Practitioners (NPs)

  • Doctors (MDs)

  • Care navigators

We’re here to help you feel confident and cared for on your road to recovery. Whether you need help coordinating care and scheduling follow up appointments, understanding your prescriptions, or finding local community services or support, we’re just a phone call away.

When is a good time to reach out to Homeward to get started?

Step 1

Take a look at the tools we offer Homeward members who engage in our post-hospital recovery program.

Step 2

Call Homeward to set up your welcome visit before you are headed into a scheduled inpatient hospital stay for a surgery or otherwise.

Establish the relationship with our doctors, nurses and care navigators so they get to know you now and be ready to support you.

Step 3

Get engaged as soon as possible with your Homeward RN who will be working with you upon discharge from the hospital.

Step 4

Just focus on healing — Homeward’s clinical team will support you and keep your doctor in the loop.
