What to expect at your Homeward Health visit
Thank you for scheduling your Homeward Health visit. We are excited to visit with you. To ensure you make the most of your visit, please review the ways to be ready for your appointment.
Confirm the date, time and location of your visit.
Have your medications and vitamins ready to review with your Care Team, as well as your driver’s license and insurance card.
Write a list of any questions you would like to discuss.
What to expect during my visit?
Your Care Navigator will review your health history so they can discuss services that will best support you. You’ll have your vitals taken, such as blood pressure, heart rate, and height and weight.
The provider will conduct a health assessment and you can:
Get answers to your health questions or concerns
Discuss tips on how to manage a chronic condition
Review your medications and any changes
Discuss any necessary preventive screenings or upcoming appointments
Your Homeward provider may join on the computer for some appointments.

What happens after my visit?
Before the Care Navigator leaves, we will schedule your next Homeward appointment to keep you on track. We will send your appointment notes to your primary care provider.

No. Homeward works with your existing doctors and specialists to coordinate your care. If you don’t have a primary care doctor or are looking for a new doctor, Homeward can get you connected with one.
